目的探讨无创正压通气（NIPPV）在治疗急性左心衰竭的应用价值。方法将62例急性左心衰竭患者随机分为两组：对照组30例用常规治疗；NIPPV组32例在给予常规治疗基础上加用NIPPV治疗，比较两组治疗前后动脉血气分析、呼吸频率、心率、血压等指标。结果NIPPV 组患者呼吸频率、心率下降，动脉血氧分压（PaO2）上升，动脉二氧化碳分压（PaCO2）下降，治疗总有效率为93.75%（30/32），对血压无明显影响；对照组治疗总有效率为70.00%（21/30），两组临床疗效比较差异有统计学意义（P＜0.05）。结论急性左心衰竭时，在常规治疗的基础上联合NIPPV治疗效果显著。

ObjectiveTo study the value of using non-invasive positive pressure ventilation (NIPPV) in the treatment of acute left-heart failure.Methods62 patients with acute left-heart failure were randomly divided into control group and NIPPV group. Control group was given routine therapy,while NIPPV group was given NIPPV in addition to routine therapy.We evaluated the change of arterial blood gas analysis,breath rate,blood pressure and heart rate of two groups’ pre-and pro therapy.ResultsBreathing rate,heart rate and PaCO2 were decreased while PaO2 increased in these patients of NIPPV group,but have no influence on their blood pressure.The total therapeutic effective rate of NIPPV group was significantly higher than that of control group ［93.75% (2/32) vs.70.00 %(9/30)］. It had significant difference between the two group (P＜0.05). ConclusionThe therapeutic effect is obvious while giving NIPPV in addition to routine therapy in the patients with acute left-heart failure.
