目的探讨颈动脉不稳定斑块患者血清同型半胱氨酸(Hcy)水平与急性脑血管意外发生的关系。方法回顾性分析300例颈动脉不稳定斑块患者的临床资料,依据是否发生急性脑血管事件分为卒中组(218例)和非卒中组(82例)。分析患者斑块厚度与血清Hcy浓度的关系;比较卒中与非卒中患者性别、年龄、饮酒、吸烟,并发糖尿病、高血压、冠心病等情况的差异;比较卒中与非卒中患者血压、空腹血糖(FBG)、血餐后2h血糖(2h-PG)、血糖化血红蛋白(HbA1c)、和血清Hcy水平;比较卒中组不同认知功能损伤患者血清Hcy水平。结果颈动脉不稳定斑块患者Hcy水平与斑块厚度呈正相关(r=0.317,P<0.05)。卒中组患者年龄、血FBG、血2h-PG、血HbA1c和血清Hcy水平显著高于非卒中组患者,差异有统计学意义(P<0.01)。卒中轻度损伤组患者血清Hcy水平显著低于中度损伤组和重度损伤组患者(P<0.01);中度损伤组患者血清Hcy水平和重度损伤组患者比较,差异无统计学意义(P>0.05)。结论高血清Hcy水平的颈动脉不稳定斑块患者,急性脑血管意外发生率可能更高且更易出现神经功能损伤。
ObjectiveTo investigate the relationship between hyperhomocysteinemia and acute cerebral vascular accident in patients with carotid unstable plaque. MethodsThe clinical data of 300 patients with carotid unstable plaque were analyzed retrospectively, and the patients were divided into stroke group (218 cases) and non stroke group (82 cases) according to the occurrence of acute cerebrovascular events. The relationship between the levels of serum homocysteine and plaque thickness was analyzed; the proportion of gender, age, drinking, smoking, complicated with diabetes, high blood pressure and coronary heart disease (CHD) were compared between the two groups; the levels of blood pressure, FBG, 2h-PG, HbA1c and Hcy were also compared between the two groups; the levels of Hcy were compared among patients with different cognitive impairment in the stroke group. ResultsThe level of serum Hcy had positive correlation with plaque thickness in patients with unstable carotid plaque(r=0.317, P<0.05). The levels of FBG, 2h-PG, HbA1c and serum Hcy in the stroke group were significantly higher than those in the non stroke group (P<0.01), patients in the stroke group were significantly older than that in the non stroke group (P<0.01). The level of serum Hcy in mild injury group was significantly lower than that in the moderate injury and severe injury group (P<0.01), but there was no significant difference in the level of serum Hcy between moderate injury group and severe injury group (P>0.05) . ConclusionsPatients with carotid unstable plaque in high level of serum Hcy might have higher incidences of acute cerebral vascular accident and neurological impairment.
