目的探讨降钙素原(Procalcitonin,PCT)与腺苷脱氨酶(adenosine deaminase,ADA)检测在小儿脑膜炎诊断中的应用价值。方法选取2013年1月至2015年1月我院儿科收治且经临床确诊的脑膜炎患儿112例作为研究组,选取同期在我院诊治的癫痫患儿100例作为对照组,检测比较两组患儿、以及不同类型小儿脑膜炎患儿血清PCT水平、脑脊液(CSF)ADA水平。结果小儿脑膜炎患儿血清PCT、CSF-ADA水平均明显高于癫痫患儿,差异具有统计学意义(P<0.01)。病毒性脑膜炎、化脓性脑膜炎患儿血清PCT水平均明显高于对照组,差异有统计学意义(P<0.05),化脓性脑膜炎患儿血清PCT水平最高,其次是病毒性脑膜炎,结核性脑膜炎最低。CSF-ADA水平按结核性脑膜炎、化脓性脑膜炎、癫痫患儿、病毒性脑膜炎顺序递减,组间比较差异有统计学意义(P<0.05)。癫痫患儿的PCT、CSF-ADA阳性率均为0;脑膜炎患儿中,化脓性脑膜炎患儿PCT阳性率最高(100.0%),其次为病毒性脑膜炎患儿(35.9%),结核性脑膜炎患儿的PCT阳性率为0;而CSF-ADA阳性率以结核性脑膜炎患儿最高(46.4%),其次为化脓性脑膜炎(20.0%),病毒性脑膜炎患儿CSF-ADA阳性率为0。结论降钙素原与腺苷脱氨酶检测有助于小儿脑膜炎的临床诊断及其鉴别诊断,值得临床推广应用。
ObjectiveTo investigate the application value of procalcitonin (Procalcitonin, PCT) and adenosine deaminase (adenosine deaminase, ADA) in diagnosis of infantile meningitis. MethodsA total of 112 children with meningitis admitted to our hospital from January 2013 to January 2015 were selected as the study group, and 100 children with epilepsy in our hospital at the same period were selected as the control group, the levels of serum PCT and CSF-ADA were compared between two groups, and the indexes mention above were also compared among different type of infantile meningitis. ResultsThe levels of serum PCT and CSF-ADA in the study group were significantly higher higher than those in the control group (P<0.05). The levels of serum PCT in the viral meningitis group, purulent meningitis group were significantly higher than those in the control group (P<0.05), in the study group, the levels of serum PCT decreased in the following order: purulent meningitis, viral meningitis, tuberculous meningitis. The levels of CSF-ADA decreased in the following order: tuberculous meningitis, purulent meningitis, epilepsy, viral meningitis group, and the difference was statistically significant among the four groups (P<0.05). The positive rate of PCT, CSF-ADA in the control group were both 0%; in the study group, the positive rate of PCT decreased in the following order: purulent meningitis (100.0%), viral meningitis (35.9%), tuberculous meningitis (0%); while the positive rate of CSF-ADA decreased in the following order: tuberculous meningitis (46.4%), purulent meningitis (20.0%),viral meningitis (0%). ConclusionsDetecting procalcitonin and adenosine deaminase is helpful for the diagnosis and differential diagnosis of meningitis in children, which is worthy of clinical application.
