目的分析2型糖尿病合并结直肠癌患者抗肿瘤治疗前后的营养状态,为更好地对患者进行营养管理提供参考。方法选取2013年6月至2014年12月在我院肿瘤中心住院治疗的2型糖尿病合并结直肠癌患者122例为研究对象,检测比较患者抗肿瘤治疗前、肿瘤治疗结束4周时的体质量、体质量指数、血红蛋白、前白蛋白水平;应用营养风险筛查工具NRS 2002对患者进行营养风险评估。结果与抗肿瘤治疗前比较,肿瘤治疗结束后4周时患者的前白蛋白、血红蛋白、体质量、体质量指数水平均显著下降(P<0.01),存在营养风险的患者的比例显著增加(P<0.01),患者NRS2002评分显著高于治疗前(P<0.01)。结论2型糖尿病合并结直肠癌患者在抗肿瘤治疗后营养水平显著降低,营养风险发生率显著增加,该对该类患者应进行动态营养筛查和评估,及时为存在营养风险的患者制定个体化方案进行营养支持,以改善患者的营养状态,提高治疗效果。
【Abstract】ObjectiveTo investigate the nutrition status of patients with type 2 diabetes mellitus complicated with colorectal cancer before and to after oncotherapy, thereby to provide the reference for nutrition Management. MethodsA total of 122 patients with type 2 diabetes mellitus complicated with colorectal cancer treated in our hospital from June 2013 to December 2016 were enrolled as the research objects,weight, body mass index(MBI), hemoglobin(HGB), Prealbumin(PA) were assessed and compared before and 4 weeks after oncotherapy, NRS 2002 was used to evaluated the nutritional risk. ResultsAfter 4 weeks of treatment, the levels of PA, HGB, MBI, weight of patients were significantly decreased compared with those before treatment (P<0.01), and the rate of nutritional risk, NRS 2002 score were significantly increased compared with those before treatment (P<0.01). ConclusionsThe incidence of nutritional risk was increased and nutrient level was decreased among patients with type 2 diabetes mellitus complicated with colorectal cancer after oncotherapy, nutritional screening should be performed and individualized program of nutrition support should be made for the patients with nutritional risk, which can improve the nutrition status of patients, increase therapeutic effect.
