目的探讨肺动脉及支气管动脉CT血管成像(CTA)在肺部可疑恶性病变诊断中的应用价值。方法对2016年5月至2017年10月因肺部可疑恶性病变在我院行肺动脉及支气管动脉CTA检查的101例患者的临床资料进行回顾性分析,按照病理确诊结果将患者分为肺癌组(63例)和肺结核组(38例),比较两组患者的左右支气管动脉(BA)、肺动脉(PA)显示情况及管径等差异。结果肺癌组患者BA清晰显影率(93.65%)高于肺结核组(52.63%),分支显示数多于肺结核组,差异有统计学意义(P<0.05);肺癌组患者左、右侧BA管径与总截面积均大于肺结核组,1级、2级PA显影阳性率高于肺结核组,差异有统计学意义(P<0.05);肺癌组患者左、右肺存在病灶直接供血动脉的比例高于肺结核组,差异有统计学意义(P<0.05)。结论肺动脉及支气管动脉CTA对肺部可疑恶性病变患者的鉴别诊断具有较高的临床应用价值。
ObjectiveTo explore the value of computed tomographic angiography (CTA) in the diagnosis of suspected malignant lesions in the lung. MethodsA retrospective analysis of the clinical data of 101 patients who underwent CTA examination of pulmonary and bronchial arteries in our hospital from May 2016 to October 2017 due to suspected malignant lesions in the lung. According to the pathological diagnosis, the patients were divided into lung cancer group (63 cases) and tuberculosis group (38 cases). The differences of left and right bronchial artery (BA), pulmonary artery (PA) and diameter were compared between the two groups. ResultsThe clear development rate of BA in patients with lung cancer (93.65%) was higher than that in tuberculosis group (52.63%), and the number of branches was higher than that in tuberculosis group, the difference was statistically significant (P<0.05). The left and right BA diameters and total cross-sectional area of lung cancer patients were higher than those of tuberculosis group. The positive rate of grade 1 and grade 2 PA development was higher than that of tuberculosis group, the difference was statistically significant (P<0.05). In the lung cancer group, the proportion of direct feeding artery in the left and right lung was higher than that in the tuberculosis group, and the difference was statistically significant (P<0.05). ConclusionCTA of pulmonary artery and bronchial artery has a high clinical value in the differential diagnosis of patients with suspected malignant lesions in the lung.
