多索茶碱联合地塞米松治疗煤工尘肺合并肺部感染临床疗效观察

目的探讨多索茶碱联合地塞米松治疗煤工尘肺合并肺部感染的临床疗效及对患者肺功能和血常规的影响。方法选取2016年2月至2017年8月我院收治的煤工尘肺合并肺部感染患者86例,随机分为观察组和对照组,每组43例。两组患者均给予常规治疗,在此基础上对照组患者应用地塞米松治疗,观察组患者在对照组治疗的基础上联合多索茶碱治疗。比较两组患者的临床疗效;检测比较两组患者的肺功能、血常规及动脉血气分析指标的变化情况。结果治疗1个月后,观察组患者的治疗总有效率(95.35%)显著高于对照组(76.74%),差异有统计学意义(P<0.05);两组患者的肺功能均明显改善,观察组患者肺功能的改善情况明显优于对照组,差异有统计学意义(P<0.05);两组患者的白细胞与中性粒细胞计数均明显降低,观察组患者的白细胞与中性粒细胞计数显著低于对照组,差异有统计学意义(P<0.05);两组患者动脉血气分析的PO2、pH值均明显升高,PCO2均明显降低,观察组患者的PO2、pH值显著高于对照组,PCO2显著低于对照组,差异有统计学意义(P<0.05)。结论多索茶碱与地塞米松联合治疗煤工尘肺合并肺部感染患者临床疗效显著,可有效改善肺功能和呼吸状况,降低肺部炎症反应。
ObjectiveTo investigate the clinical effect of doxofylline combined with dexamethasone in the treatment of coal-workers with pneumoconiosis complicated with pulmonary infection and its influences on lung function and blood routine test. Methods86 cases of coal-workers with pneumoconiosis complicated with pulmonary infection treated in our hospital from February 2016 to August 2017 were selected and randomly divided into observation group and control group, with 43 cases in each group. The two groups were given routine treatment. On this basis, the control group were treated with dexamethasone, the observation group were treated with doxofylline on the basis of of the therapeutic method of control group. The clinical curative effect, lung function, blood routine test and arterial blood gas analysis were compared between the two groups. ResultsAfter 1 month treatment, the total effective rate of the observation group was 95.35%, which was significantly higher than 76.74% of the control group (P<0.05); the lung function of the two groups after the treatment were higher than those before the treatment. The observation group was higher than the control group (P<0.05). The white blood cell and neutrophil counts of the two groups were significantly reduced. The white blood cell and neutrophil counts of the observation group were significantly lower than the control group (P<0.05). The blood oxygen partial pressure (PO2) and pH in the two groups were higher than before the treatment (P<0.05), the blood carbon dioxide partial pressure (PCO2) was lower than that before treatment (P<0.05), and the PO2 and pH in the observation group were higher than those in the control group (P<0.05), the PCO2 was lower than that in the control group (P<0.05). ConclusionThe combination of doxofylline and dexamethasone in the treatment of coal-workers with pneumoconiosis complicated with pulmonary infection is clinically effective, which can effectively improve lung function and respiratory status, reduce lung inflammation.
