CK19 and TTF-1 expression in lymphoid tissues of patients with non-small cell lung cancer and its clinical significance

目的探讨非小细胞肺癌患者淋巴组织的细胞角蛋白19(CK19)、甲状腺转录因子1(TTF-1)表达情况及其临床意义。方法选取2002年1月至2008年12月我院确诊的非小细胞肺癌Ⅱ期患者68例作为研究对象,收集患者手术切除的淋巴结标本进行HE染色和免疫组化染色,将HE染色和免疫组化染色检测结果任一阳性的患者设为淋巴转移组,HE染色和免疫组化染色检测结果均为阴性的患者设为非淋巴转移组。采用免疫组化染色法检测淋巴转移组和非淋巴转移组患者的CK19、TTF-1表达情况。将淋巴组织CK19、TTF-1检测结果均为阳性的患者设为微转移组,将CK19、TTF-1检测结果单阴性或全阴性的患者设为无微转移组,随访3年,比较微转移组和无微转移组患者的3年生存率。结果淋巴结转移组患者淋巴组织的TTF-1、CK19阳性表达率明显高于无淋巴结转移组患者,差异有统计学意义(P<0.05)。淋巴结微转移组患者的3年生存率(57.14%)明显低于无微转移组患者(88.89%),差异有统计学意义(P<0.05)。结论非小细胞肺癌患者淋巴组织中CK19、TTF-1的表达与患者淋巴结转移和术后生存的关系密切,与肺癌发生微转移有关。
ObjectiveTo investigate the expression of cytokeratin 19 (CK19) and thyroid transcription factor 1 (TTF-1) in lymphoid tissues of patients with non-small cell lung cancer and its clinical significance. MethodsA total of 68 patients diagnosed as stage II non-small cell lung cancer in our hospital from January 2002 to December 2008 were selected as subjects. The lymph node specimens surgically removed from the patients were collected, and HE staining and immunohistochemical staining were performed. The patients with positive HE staining and immunohistochemical staining results were set as lymphatic metastasis group, and patients with negative HE staining and immunohistochemical staining were set as non-lymphatic metastasis group. Immunohistochemical staining was used to detect the expression of CK19 and thyroid TTF-1 in patients with lymphatic metastasis and non-lymphatic metastasis. Patients with positive lymph node CK19 and TTF-1 in lymph tissue were set as micrometastases group, and patients with negative CK19 and/or TTF-1 were set as non-micrometastases group. After 3 years of follow-up, the 3-year survival rate of patients in the micrometastatases group and the non-micrometastases group was compared. ResultsThe positive expression rates of TTF-1 and CK19 in lymphoid tissue of patients with lymphatic node metastasis were significantly higher than those without lymphatic node metastasis (P<0.05). The 3-year survival rate of patients with lymphatic node micrometastasis (57.14%) was significantly lower than those without micrometastasis (88.89%), and the difference was statistically significant (P<0.05). ConclusionThe expression of CK19 and TTF-1 in lymphoid tissues of patients with non-small cell lung cancer is closely related to lymph node metastasis and postoperative survival, and which is associated with micrometastasis in lung cancer.
