目的探讨深圳市宝安区健康妊娠妇女血清肝肾功能指标水平的变化情况并建立正常值参考区间。方法选取2017年11月至2018年8月在深圳市宝安区妇幼保健院及社区康复中心接受孕检的孕4~40周健康妊娠妇女为研究对象,根据她们的年龄、孕期进行分组,采血进行肝、肾功能指标水平检测分析,根据检测结果建立深圳市宝安区健康妊娠妇女血清肝肾功能指标水平的正常值参考区间。结果不同年龄健康孕妇肝肾功能指标比较差异有统计学意义,低年龄组(19~32岁)孕妇血清碱性磷酸酶(ALP)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆汁酸(TBA)、重碳酸盐(HCO3-)、肌酐(Cr)、尿酸(UA)水平低于高年龄组(33~45岁),血清总蛋白(TP)、白蛋白(ALB)、总胆红素(TBIL)、尿素(Ur)水平高于高年龄组,差异有统计学意义(P<0.05)。不同孕期健康孕妇肝肾功能指标水平比较差异有统计学意义(P<0.05),孕妇血清ALP、UA水平随孕周增加而升高,血清TP、ALB、TBIL、HCO3-、Cr水平随孕周增加而降低。结论不同年龄、不同孕期健康妊娠妇女血清肝肾功能指标水平存在较大差异,建立不同地区妊娠期女性肝肾功能指标水平参考区间对妊娠期疾病的诊断、预防及治疗具有重要意义。
ObjectiveTo explore the changes of serum liver and kidney function indicators in healthy pregnant women in Baoan District of Shenzhen City and establish a reference range of normal value. MethodsThe healthy pregnant women who were pregnant for 4 to 40 weeks from November 2017 to August 2018 in Shenzhen Baoan District Maternal and Child Healthcare Hospital and Community Rehabilitation Center were selected as subjects. Grouping by their age and pregnancy, the blood samples were collected and analyzed for liver and kidney function indicators. According to the test results, the normal reference range of serum liver and kidney function indicators of healthy pregnant women in Baoan District of Shenzhen City was established. ResultsThere were statistically significant differences in liver and kidney function indexes between healthy women of different ages. Serum alkaline phosphatase (ALP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bile acid (TBA), bicarbonate Salt (HCO3-), creatinine (Cr), uric acid (UA) levels in pregnant women of low-age group (19-32 years old) were lower than those in high age group (33-45 years old). Serum total protein (TP), albumin (ALB), total bilirubin (TBIL), urea (Ur) levels were higher than those in high age group, and the difference was statistically significant (P<0.05). There were significant differences in liver and kidney function between healthy pregnant women with different pregnancy stages (P<0.05). The levels of serum ALP and UA increased with gestational age. The levels of serum TP, ALB, TBIL, HCO3-, Cr decreased with gestational age. ConclusionThere are significant differences in serum liver and kidney function indicators between healthy women of different ages and different pregnancy stages. Establishing a reference interval for liver and kidney function indicators in pregnant women in different regions is of great significance for the diagnosis, prevention and treatment of gestational diseases.
