目的分析支气管肺泡灌洗治疗呼吸机相关性肺炎(VAP)患者的临床价值。方法对2014~2017年在我院重症监护室(ICU)被诊断为VAP的患者53例进行支气管肺泡灌洗治疗(3次以上),观察患者的临床疗效及相关指标的变化。结果纳入研究患者共53例,治愈4例,有效43例,死亡6例,治疗总有效率为88.68%,患者死亡原因为多器官功能衰竭。53例患者的灌洗液均检出病原菌,其中检查革兰氏阴性菌患者47例(88.68%),检出革兰氏阳性菌患者5例(9.43%),检出真菌患者1例(1.89%)。耐药试验结果显示,50例(94.3%)患者存在多重耐药菌感染。治疗后,患者的PCO2、氧合指数、白细胞、CRP水平均明显改善(P<0.05),而pH值、体温与治疗前比较,差异无统计学意义(P>0.05)。患者均不出现恶性心律失常、窒息及大咯血等严重不良反应。结论对VAP患者早期进行支气管肺泡灌洗治疗,能够更准确地获得病原学检测结果,有利于正确选取敏感抗生素进行治疗,临床治疗效果显著,治疗安全性高。
ObjectiveTo analyze the clinical value of bronchoalveolar lavage on patients with ventilator-associated pneumonia (VAP). MethodsBronchoalveolar lavage was performed on 53 patients with VAP in our intensive care unit (ICU) from 2014 to 2017, and the clinical efficacy and related indicators were observed. ResultsA total of 53 patients were enrolled in the study, in which 4 were cured, 43 were effective, and 6 died. The total effective rate was 88.68%, and the cause of death was multiple organ failure. Pathogens were detected in the lavage fluid of 53 patients, of which 47(88.68%) were Gram-negative bacteria, 5 (9.43%) were Gram-positive bacterian and 1(1.89%)was fungal. The result of drug resistance test showed that 50 (94.3%) patients had multi-drug resistant organism infection. After treatment, the patients′ PCO2, oxygenation index, white blood cells and CRP level were significantly improved (P<0.05), and there was no statistically significant difference in pH value and body temperature before and after treatment (P>0.05). No patients had serious adverse reactions including malignant arrhythmia, asphyxia and massive hemoptysis. ConclusionEarly bronchoalveolar lavage treatment on patients with VAP can obtain the results of pathogenic detection more accurately, which is conducive to the correct selection of sensitive antibiotics for treatment, and the clinical treatment has significant effect and high safety.
