目的比较分析计算机断层扫描(CT)、磁共振成像(MRI)检查诊断早期腔隙性脑梗死的临床应用价值。方法以颅脑MRI的弥散加权序列(DWI)检查作为诊断腔隙性脑梗死的金标准,分别采用CT常规扫描及MRI扫描的常规序列对80例患者进行检查诊断,比较分析二者对不同病程早期腔隙性脑梗死的检出率以及对不同大小病灶的检出率。结果CT与MRI检查腔隙性脑梗死的检出率分别为33.75%和97.50%,两者比较差异有统计意义(P<0.05)。对病程<6 h和6~24 h的患者,MRI检查腔隙性脑梗死检出率分别为96.88%和95.45%,明显高于CT检查,两者比较差异有统计意义(P<0.05);对病程为24~72 h的患者,CT和MRI检查腔隙性脑梗死的检出率比较差异无统计学意义(P>0.05)。在80例腔隙性脑梗死患者中,CT与MRI检查检出的病灶数分别为96个和235个,两者的病灶检出率(分别为39.34%和96.31%)比较差异有统计意义(P<0.05)。对病程为<6 h和6~24 h的患者,CT与MRI检查的病灶检出率比较差异有统计意义(P<0.05);对病程为24~72 h的患者,CT和MRI检查的病灶检出率比较差异无统计学意义(P>0.05)。结论MRI对早期腔隙性脑梗死的检出率以及对较小病灶的检出率均明显高于CT检查,对早期腔隙性脑梗死的诊断具有更高的临床应用价值。
ObjectiveTo compare and analyze the clinical application value of computed tomography (CT) and magnetic resonance imaging (MRI) in diagnosis of early lacunar infarction. MethodsDiffusion-weighted sequence (DWI) examination of brain MRI is used as the gold standard for diagnosing lacunar infarction, and 80 patients were examined and diagnosed by routine CT scan and MRI scan. The detection rates of early lacunar infarction in different stages and different size lesions were compared and analyzed. ResultsThe detection rates of lacunar infarction by CT and MRI were 33.75% and 97.50%, respectively, and the difference was statistically significant (P<0.05). On the patients with course of disease <6h and 6h-24h, the detection rates of lacunar infarction on MRI were 96.88% and 95.45%, respectively, which were significantly higher than CT examination, and the difference was statistically significant (P<0.05). There was no statistically significant difference in the detection rate of lacunar infarction between CT and MRI on patients with course of disease 24h-72h (P>0.05). In the 80 patients with lacunar infarction, the number of lesions detected by CT and MRI was 96 and 235, respectively. The difference in the detection rate of the lesions (39.34% and 96.31%, respectively) was statistically significant (P<0.05). The difference in the detection rate of CT and MRI on patients with course of disease <6 h and 6h-24h was statistically significant (P<0.05), and was not statistically significant on patients with course of disease 24h-72h (P>0.05). ConclusionThe detection rates of early lacunar infarction and small lesions by MRI were significantly higher than those by CT, and MRI has higher clinical value for early lacunar infarction diagnosis.
