目的探讨妊娠期糖尿病(GDM)孕妇孕期体重增长对胎儿生长发育和妊娠结局的影响。方法选取2018年6~12月我院收治的GDM孕妇150例及其所产新生儿为研究对象,根据孕妇孕期体重增长情况将孕妇及其所产新生儿分为体重增长不足组,体重增长正常组,体重增长过多组。检测比较三组患者胎儿腹围正常增长率;比较三组患者所产新生儿的Apgar评分;比较三组患者的低出生体重儿和巨大儿发生率。结果在不同孕周,三组患者的胎儿腹围增长正常率均表现为体重增长正常组>体重增长过多组和体重增长不足组,差异有统计学意义(P<0.05)。体重增长不足组患者的低出生体重新生儿发生率显著高于体重增长过多组和体重增长正常组,差异有统计学意义(P<0.05);体重增长过多组患者的巨大儿发生率显著高于体重增长正常组和体重增长不足组,差异有统计学意义(P<0.05);三组患者的新生儿Apgar评分表现为体重增长过多组<体重增长不足组<体重增长正常组,差异有统计学意义(P<0.05)。结论妊娠期糖尿病孕妇孕期的体重增长与其胎儿的腹围增长以及妊娠结局密切相关,保持孕妇适宜的体重增长对促进胎儿的正常发育、降低不良妊娠结局发生率具有重要的意义。
ObjectiveTo investigate the effect of gestational weight gain on fetal growth and pregnancy outcome in pregnant women with gestational diabetes mellitus (GDM). MethodsA total of 150 pregnant women with GDM and their newborns admitted to our hospital from June to December 2018 were selected as the research objects. According to the weight gain of pregnant women during pregnancy, pregnant women and their newborns were divided into underweight gain group, normal weight gain group, and excessive weight gain group. The normal growth rate of fetal abdomen circumference, the Apgar score of the newborns and the incidence of low birth weight infants and macrosomia were compared between the three groups. ResultsAt different gestational weeks, the normal rate of fetal abdomen circumference in the three groups in a descending order was the normal weight gain group, excessive weight gain group and underweight gain group, with statistically significant difference (P<0.05). The incidence of low birth weight infants in the underweight gain group was significantly higher than that in the excessive weight gain group and the normal weight gain group, with statistically significant difference (P<0.05). The incidence of macrosomia in the excessive weight gain group was significant higher than that in the normal weight gain group and underweight gain group, with statistically significant difference (P<0.05). The Apgar score in the three groups in a descending order was the normal weight gain group, underweight gain group, and excessive weight gain group, with statistically significant difference (P<0.05). ConclusionThe gestational weight gain of pregnant women with gestational diabetes mellitus is closely related to their fetal abdomen circumference and pregnancy outcomes. Maintaining a suitable gestational weight gain of pregnant women is of great significance in promoting normal fetal development and decreasing the incidence of adverse pregnancy outcome.
