目的观察丁苯酞联合阿替普酶静脉溶栓治疗急性脑梗死(ACI)患者的疗效及对神经递质水平、神经功能的影响。方法选择2017年1月至2019年6月在我院住院治疗的ACI患者78例为研究对象,采用随机数字法分为对照组和观察组,每组39例。对照组患者给予阿替普酶溶栓治疗,观察组给予阿替普酶联合丁苯酞治疗。两组患者均连续治疗2周。比较两患者的临床疗效、血多巴胺和5-羟色胺水平、Barthel指数(BI评分)以及不良反应发生情况。结果观察组患者的治疗总有效率(94.87%)显著高于对照组(76.92%),临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者的血多巴胺、5-羟色胺水平、BI评分比较,差异无统计学意义(P>0.05);治疗2周后,观察组患者的血多巴胺、5-羟色胺水平以及BI评分均显著高于对照组,差异有统计学意义(P<0.05)。治疗期间,两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论丁苯酞联合阿替普酶静脉溶栓治疗ACI患者临床效果确切,治疗安全性高,能显著提高患者的神经递质水平,改善患者神经功能,提高患者生活自理能力。
ObjectiveTo observe the efficacy of butylphthalide combined with alteplase intravenous thrombolysis in treating patients with acute cerebral infarction (ACI) and its effect on neurotransmitter level and nerve function. MethodsA total of 78 patients with ACI hospitalized in our hospital from January 2017 to June 2019 were selected as the research objects, and were divided into control group and observation group by random number method, with 39 cases in each group. The control group was given alteplase thrombolytic therapy, and the observation group was treated with alteplase combined with butylphthalide, for two-week continuous treatment in both groups. The clinical efficacy, blood dopamine and serotonin levels, Barthel index (BI) scores, and incidence of adverse reactions were compared between the two groups. ResultsThe total effective rate of treatment in the observation group was significantly higher than that in the control group (94.87% vs. 76.92%), and the clinical efficacy was superior to that in the control group, with statistically significant differences (P<0.05). There were no statistically significant differences in dopamine, serotonin levels and BI scores between the two groups before treatment (P>0.05). After 2 weeks of treatment, the levels of dopamine and serotonin, as well as BI scores in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). During treatment, there was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). ConclusionButylphthalide combined with alteplase intravenous thrombolysis has a definite clinical effect in ACI, with high safety of treatment, which can significantly increase the level of neurotransmitter, improve nerve function, and thus improve self-care ability of daily living in patients.
