血清性激素水平与初诊未治精神分裂症患者认知功能的关系分析

目的探讨血清性激素水平与初诊未治精神分裂症患者认知功能的关系。方法选取2013年5月至2016年3月在我院就诊的初诊未治精神分裂症患者98例作为观察组,选择同期在我院进行健康体检的志愿者80例作为对照组。采用威斯康星卡片分类测验(WCST)评估两组研究对象的认知功能;采用持续操作测验(CPT)评估两组研究对象的注意力;采用精神分裂症认知功能成套测验(MCCB)评估观察组患者的认知功能,测试内容包括视觉学习评估(BVMT)、持续操作-相同配对(CPT-IP)、韦式计量量表(第三版,WMS-Ⅲ)测评、霍普金斯词语学习测验(HVLT)、神经心理评估成套测验(NTB)、情绪管理(MSCEIT)评价、质量控制(SOP)7项。检测比较两组研究对象的性激素水平;对精神分裂症患者的性激素水平与其阳性和阴性症状量表(PANSS)评分、MCCB评分进行Pearson相关性分析。结果观察组患者的WCST总应答数、随机错误数、持续错误数评分显著高于对照组,WCST正确应答数、完成分类数评分及CPT总分显著低于对照组,差异有统计学意义(P<0.05)。观察组男性患者的催乳素水平明显高于对照组男性,睾酮、雌二醇和促黄体生成素水平明显低于对照组男性,差异有统计学意义(P<0.05)。观察组女性患者的催乳素、孕酮水平明显高于对照组女性(P<0.05),睾酮和雌二醇水平明显低于对照组女性,差异有统计学意义(P<0.05)。观察组男性患者的催乳素水平与其WMS-Ⅲ(r=-0.290)和NTB(r=-0.351)评分呈负相关,睾酮水平与BVMT(r=0.610)和NTB(r=0.560)评分呈正相关,雌二醇水平与BVMT(r=0.296)、NTB(r=0.410)、SOP(r=0.355)评分呈正相关。女性精神分裂症患者的催乳素水平与BVMT(r=-0.198)、WMS-Ⅲ(r=-0.311)、NTB(r=-0.382)、MSCEIT(r=-0.379)评分呈负相关,促卵泡性腺激素水平与MSCEIT(r=-0.219)评分呈负相关,雌二醇水平与PANSS评分(r=-0.399)呈负相关,雌二醇水平与HVLT(r=0.504)、MSCEIT(r=0.399)、SOP(r=0.410)评分呈正相关。结论初诊未治精神分裂症患者的认知功能降低,患者的性激素水平与健康人群比较存在差异,催乳素水平升高,睾酮、雌二醇水平降低可能与精神分裂症的发病有关。
ObjectiveTo explore the relationship between serum sex hormone levels and cognitive function of newly diagnosed untreated schizophrenia. MethodsA total of 98 cases of newly diagnosed untreated schizophrenia treated in our hospital from May 2013 to March 2016 were selected as observation group, and 80 volunteers undergoing health examinations in our hospital during the same period were selected as control group. The Wisconsin Card Sorting Test (WCST) was used to assess the cognitive function, with the Continuous Performance Test (CPT) to assess the attention in both groups, and with MATRICS Consensus Cognitive Battery (MCCB) to evaluate cognitive function of the observation group, including 7 items as for Brief Visuospatial Memory Test (BVMT), Continuous Performance Test-Identical Pairs (CPT-IP), Wechsler memory scale (the third edition, WMS-Ⅲ) assessment, Hopkins Verbal Learning Test (HVLT), Neuropsychological Test Battery (NTB), Mayer-Salovey-Caruso Emotional Intelligence Test (MSCEIT), Standard Operation Procedure (SOP). The sex hormone levels were detected and compared between the two groups. Pearson correlation analysis was performed to analyze the sex hormone levels of patients with schizophrenia and their positive and negative symptom scale (PANSS) scores and MCCB scores. ResultsCompared to the control group, the observation group yielded higher numbers of WCST total response and random error, as well as continuous error number scores, whereas lower number of WCST correct response, scores of completed sorting number, and CPT total scores, with statistically significant differences (P<0.05). Male in the observation group obtained a higher prolactin level, whereas lower levels of testosterone, estradiol, and luteinizing hormone in comparison with male in the control group; meanwhile, female in the observation group obtained higher prolactin and progesterone levels, whereas lower levels of testosterone and estradiol compared to female in the control group, with statistically significant differences (P<0.05). Male patients in the observation group obtained negative correlations between prolactin and WMS-Ⅲ (r=-0.290), NTB (r=-0.351) scores, whereas with positive correlations between testosterone level and BVMT (r=0.610), NTB (r = 0.560) scores, and with positive correlations between estradiol level and BVMT (r=0.296), NTB (r=0.410), SOP (r=0.355) scores. Prolactin level of female schizophrenia patients was negatively correlated with BVMT (r=-0.198), WMS-Ⅲ (r=-0.311), NTB (r=-0.382), MSCEIT (r=-0.379) scores, follicle stimulating hormone level was negatively correlated with MSCEIT (r=-0.219) score, and estradiol level was negatively correlated with PANSS (r=-0.399), whereas estradiol level was positively correlated with HVLT (r=0.540), MSCEIT (r=0.399), SOP (r=0.410) scores. ConclusionCognitive function decreases in newly diagnosed untreated patients with schizophrenic, and the levels of sex hormone in patients are different from healthy people. Increased prolactin levels, decreased testosterone and estradiol levels may be related to the onset of schizophrenia.
