联合抗阻-有氧运动对糖尿病前期患者胰岛素抵抗的影响▲

目的探讨联合抗阻-有氧运动对糖尿病前期患者胰岛素抵抗、胰岛素敏感性、糖尿病发生率等的影响。方法选取2015年11月至2016年2月在广西河池市中医医院体检科被确诊为糖尿病前期的患者90例为研究对象,采用随机数字法分为有氧运动组、抗阻运动组、联合抗阻-有氧运动组,每组30例。三组患者分别接受有氧运动训练、抗阻运动训练、联合抗阻-有氧运动训练,持续训练2年。比较三组患者训练前、训练2年后的胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI)以及训练期间的糖尿病发病情况。结果训练2年后,三组患者的HOMA-IR水平均显著降低,联合抗阻-有氧运动组患者的HOMA-IR水平显著低于有氧运动组及抗阻运动组,差异有统计学意义(P<0.05);三组患者的ISI水平均显著升高,联合抗阻-有氧运动组患者的ISI水平显著高于有氧运动组及抗阻运动组,差异有统计学意义(P<0.05)。训练期间,联合抗阻-有氧运动组患者的糖尿病发生率(6.7%)显著低于有氧运动组(30.0%)及抗阻运动组(26.7%),差异有统计学意义(P<0.05)。结论联合抗阻-有氧运动可有效降低糖尿病前期患者的胰岛素抵抗水平,提高患者机体对胰岛素的敏感性,降低患者的糖尿病发生率。
ObjectiveTo explore the effects of combined resistance-aerobic exercises on insulin resistance, insulin sensitivity, and incidence of diabetes for patients with prediabetes. MethodsA total of 90 patients who were diagnosed as prediabetes in the Department of Physical Examination of Traditional Chinese Medicine Hospital of Hechi, Guangxi from November 2015 to February 2016 were selected as the research objects, and they were divided into aerobic exercise group, resistance exercise group and combined resistance-aerobic exercise group by the random number method, with 30 cases in each group. The three groups received aerobic exercise training, resistance exercise training, combined resistance-aerobic exercise training, for two-year continuous training. The pre-training and two-year-after-training homeostasis model assessment insulin resistance (HOMA-IR), insulin sensitivity index (ISI) ,and the onset of diabetes during training were compared between the two groups. ResultsAfter 2 years of training, the HOMA-IR levels of the three groups significantly decreased, and the levels mentioned above in the combined resistance-aerobic exercise group were significantly lower than those in the aerobic exercise group and the resistance exercise group, with statistically significant differences (P<0.05). The ISI level of the three groups significantly increased, and the ISI level of the combined resistance-aerobic exercise group was significantly higher than that of the aerobic exercise group and the resistance exercise group, with a statistically significant difference (P<0.05). During training, the incidence of diabetes in the combined resistance-aerobic exercise group (6.7%) was significantly lower than that in the aerobic exercise group (30.0%) and the resistance exercise group (26.7%), and the difference was statistically significant (P<0.05 ). ConclusionCombined resistance-aerobic exercises can effectively decrease the levels of insulin resistance for patients with prediabetes, increase insulin sensitivity, and decrease the incidence of diabetes in patients.
