CA19-9和CA125在甲状腺乳头状癌中的表达及临床意义▲

目的探讨甲状腺乳头状癌中糖类抗原19-9(CA19-9)、糖类抗原125(CA125)的表达情况及其临床意义。方法选择2017年6月至2018年6月在我院就诊的首诊甲状腺乳头状癌患者和甲状腺良性病变患者各30例为研究对象。两组患者均给予手术治疗,采用免疫组化Max Vision两步法检测两组患者病变组织中的CA125、CA19-9表达情况。结果甲状腺乳头状癌组患者病变组织的CA125、CA19-9阳性表达率分别为66.7%、80.0%,甲状腺良性病变组患者病变组织的CA125、CA19-9阳性表达率分别为20.0%、26.7%,两组比较差异有统计学意义(P<0.05)。CA19-9阳性病变组织表现为细胞腔缘或胞膜呈线条状着色,多为片状;CA125阳性病变组织表现为细胞腔缘或顶端胞膜呈线条状,大部分胞质着色较弱。不同性别、年龄、肿瘤直径、淋巴结转移情况、临床TNM分期、瘤结节数目的甲状腺乳头状癌患者,其病变组织的CA125、CA19-9阳性表达率比较差异均无统计学意义(P>0.05)。结论甲状腺乳头状癌中CA125、CA19-9呈现高表达状态,对甲状腺乳头状癌和甲状腺良性病变的鉴别诊断具有重要的参考价值。
ObjectiveTo explore the expressions and clinical significance of carbohydrate antigen 19-9 (CA19-9) and carbohydrate antigen 125 (CA125) in papillary thyroid carcinoma. MethodsA total of 30 patients firstly diagnosed as papillary thyroid carcinoma and 30 patients with benign thyroid lesions who were treated in our hospital from June 2017 to June 2018 were selected as the research subjects. Patients in both groups were given surgical treatment. The expressions of CA125 and CA19-9 in the lesion tissues were detected in both groups by immunohistochemical Max Vision two-step method. ResultsThe positive expression rates of CA125 and CA19-9 in the lesion tissues of patients in the papillary thyroid carcinoma group were 66.7% and 80.0%, respectively, whereas the positive expression rates mentioned above in the benign thyroid lesions group were 20.0% and 26.7%, respectively, with statistically significant differences (P<0.05). CA19-9 positive lesion tissues showed linear coloring of the cell lumen edge or cell membrane, and mainly presented as platy, whereas CA125 positive lesion tissues showed the linear shape of the cell lumen edge or the top of the cell membrane, and most periplasm interpreted weak coloring. There were no statistically significant differences in the positive expression rates of CA125 and CA19-9 among patients with papillary thyroid carcinoma in different genders, age, tumor diameter, lymph node metastasis, clinical TNM stages, and the number of tumor nodules (P>0.05). ConclusionCA125 and CA19-9 interpret high expressions in papillary thyroid carcinoma, which has an important reference value for the differential diagnosis of papillary thyroid carcinoma or benign thyroid lesions.
