目的探讨甲状腺功能亢进症(甲亢)伴心房颤动(房颤)患者的凝血功能与其甲状腺激素水平的关系。方法选取2018年9月至2019年8月在本院就诊的甲亢伴房颤患者70例(观察组)和同期在本院就诊的单纯甲亢患者70例(对照组)为研究对象,检测比较两组患者的促甲状腺素(TSH)水平、游离三碘甲状腺原氨酸(FT3)水平、游离甲状腺素(FT4)水平、凝血酶时间(TT)、纤维蛋白原(FIB)水平、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)。对甲亢伴房颤患者的凝血功能指标水平与其甲状腺激素水平的关系进行Pearson相关分析。结果两组患者的TSH、FT3、FT4水平比较差异无统计学意义(P>0.05)。观察组患者的PT水平显著高于对照组,差异有统计学意义(P<0.05)。两组患者的TT、FIB、APTT水平比较差异无统计学意义(P>0.05)。甲亢伴房颤患者的APTT与FT3、FT4呈负相关(r=-0.223,r=-0.250)。结论甲亢伴发房颤患者血液处于高凝状态,其凝血功能状态以及甲状腺激素水平与单纯甲亢患者的差异不大,患者的凝血指标水平与其甲状腺激素水平无明显相关性。
ObjectiveTo explore the relationship between coagulation function and thyroid hormone levels in patients with hyperthyroidism complicated with atrial fibrillation. MethodsA total of 70 patients with hyperthyroidism complicated with atrial fibrillation (observation group) treated in our hospital from September 2018 to August 2019, along with 70 patients with simple hyperthyroidism (control group) also treated in our hospital during the same period were selected as the research objects. The levels of thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4), thrombin time (TT), fibrinogen (FIB), activated partial thromboplastin time (APTT), prothrombin time (PT) were detected and compared between the two groups. Pearson correlation analysis was performed on the relationship between the coagulation function indicators levels and the thyroid hormone levels in patients with hyperthyroidism complicated with atrial fibrillation. ResultsThere were no statistically significant differences in the levels of TSH, FT3, and FT4 between the two groups (P>0.05). The PT level of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). There were no statistically significant differences in the levels of TT, FIB, and APTT between the two groups (P>0.05). The APTT of patients with hyperthyroidism complicated with atrial fibrillation was negatively correlated with FT3 and FT4 (r=-0.223, r=-0.250). ConclusionThe blood of patients with hyperthyroidism complicated with atrial fibrillation is in a state of hypercoagulability, and the differences between coagulation function and thyroid hormone levels in the above-mentioned patients are not significant as compared with those with simple hyperthyroidism. There is no significant correlation between the coagulation indexes levels and thyroid hormone levels in patients.
