目的探讨高通量血液透析对慢性肾衰竭尿毒症患者肾功能及免疫功能的影响。方法选择2019年6月至2020年6月我院收治的慢性肾衰竭尿毒症患者122例,采用随机数字法分为两组,每组61例。对照组患者给予常规血液透析治疗,研究组患者给予高通量血液透析治疗,疗程28 d。比较两组患者的临床疗效,检测比较两组患者治疗前后的肾功能、免疫球蛋白水平以及并发症发生情况。结果1个疗程结束后,研究组患者的治疗总有效率(96.72%)显著高于对照组(85.25%),差异有统计学意义(P<0.05);研究组患者的血尿素氮(BUN)、血肌酐(SCr)、β2-微球蛋白(β2-MG)水平显著低于对照组,尿素氮清除指数(Kt/V)显著高于对照组,差异有统计学意义(P<0.05);研究组患者的免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平显著高于对照组,差异有统计学意义(P<0.05)。治疗期间,研究组患者皮肤瘙痒、心脏病等并发症发生率(3.28%)显著低于对照组(13.11%),差异有统计学意义(P<0.05)。结论高通量血液透析能够改善慢性肾衰竭尿毒症患者的肾功能,提高患者的免疫系统功能,提高临床疗效,减少并发症的发生。
ObjectiveTo investigate the effect of high flux hemodialysis on renal function and immune function in uremic patients with chronic renal failure. MethodsA total of 122 uremic patients with chronic renal failure admitted to our hospital from June 2019 to June 2020 were selected, and they were divided into two groups by the random number method, with 61 cases in each group. The control group was treated with conventional hemodialysis, and the study group was treated with high flux hemodialysis, for a 28-day treatment course. The clinical efficacy was compared between the two groups. The renal function, immunoglobulin level and complications of the two groups were detected and compared. ResultsAfter a course of treatment, the total effective rate of treatment in the study group (96.72%) was significantly higher than that in the control group (85.25%), with a statistically significant difference (P<0.05). The blood urea nitrogen (BUN), serum creatinine (SCr) and β2-microglobulin (β2-MG) levels of the study group were significantly lower than those of the control group, while the urea nitrogen clearance index (Kt/V) of the study group was significantly higher than that of the control group, with statistically significant differences (P<0.05). The levels of immunoglobulin A (IgA), immunoglobulin M (IgM), and immunoglobulin G (IgG) in the study group were significantly higher than those in the control group, with statistically significant differences (P<0.05). During the treatment, the incidence of complications including skin itching and heart disease etc. in the study group (3.28%) was significantly lower than that in the control group (13.11%), with a statistically significant difference (P<0.05). ConclusionHigh flux hemodialysis can improve the renal function of uremic patients with chronic renal failure, improve the immune system function and clinical efficacy, and reduce the occurrence of complications in patients.
