目的探讨阿奇霉素联合丙种球蛋白治疗肺炎支原体肺炎(MMP)患儿的临床效果及对患儿免疫功能的影响。方法选择2018年1月至2020年4月我院收治的MMP患儿90例,采用随机数字法分为联合组和单药组,每组45例。单药组患儿给予阿奇霉素治疗,联合组患儿给予阿奇霉素联合丙种球蛋白治疗,疗程5 d。比较两组患儿的临床疗效、症状体征改善时间、炎症和免疫功能改善状况以及不良反应发生情况。结果治疗5 d,联合组患儿的治疗总有效率(95.6%)显著高于单药组(80.0%),差异有统计学意义(P<0.05)。联合组患儿的咳嗽缓解时间、发热缓解时间、肺部影像学改善时间、肺部啰音消失时间均显著短于对照组,差异有统计学意义(P<0.05)。治疗前,两组患儿的C反应蛋白 (CRP)、免疫球蛋白(Ig)A、IgM、IgG水平比较,差异无统计学意义(P>0.05);治疗5 d后,联合组患儿的CRP水平显著低于单药组,IgA、IgM、IgG水平显著高于单药组,差异有统计学意义(P<0.05)。治疗期间,两组患儿的不良反应发生率比较,差异无统计学意义(P>0.05)。结论阿奇霉素联合丙种球蛋白治疗小儿肺炎支原体肺炎临床疗效显著,能够有效缓解患儿的炎症反应、改善免疫功能,促进患儿康复。
ObjectiveTo investigate the clinical effect of azithromycin combined with gamma globulin in the treatment of children with mycoplasma pneumoniae pneumonia (MMP) and its influences on immune function. MethodsA total of 90 children with MMP admitted to our hospital from January 2018 to April 2020 were selected and divided into combined group and single drug group by the random number method, with 45 cases in each group. The children in the single drug group were treated with azithromycin, whereas the children in the combined group were treated with azithromycin combined with gamma globulin, for a 5-day treatment course. The clinical efficacy, improvement time of symptoms and signs, improvement of inflammation and immune function, and occurrence of adverse reactions were compared between the two groups. ResultsAfter 5 days of treatment, the total effective rate of the combined group (95.6%) was significantly higher than that of the single drug group (80.0%), with a statistically significant difference (P<0.05). The children in the combined group obtained shorter time of cough relief, fever relief, pulmonary imaging improvement, and lung rales disappear as compared with the single drug group, with statistically significant differences (P<0.05). Before treatment, there were no statistically significant differences in the levels of C-reactive protein (CRP), immunoglobulin (Ig) A, IgM and IgG between the two groups (P>0.05). After 5 days of treatment, the combined group yielded a lower level of CRP, whereas higher levels of IgA, IgM, and IgG as compared to the single drug group (P<0.05). During the treatment, there was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). ConclusionAzithromycin combined with gamma globulin has prominent clinical efficacy for the treatment of children with MPP. It can effectively alleviate inflammatory responses of children, improve immune function, and promote recovery of children.
