目的探讨精液不液化不育患者的精子DNA碎片指数(DFI)与其精子常规参数、精子顶体酶活性的关系及其对体外受精结局的影响。方法选取2018年12月至2020年5月在我院就诊的精液不液化不育患者106例为观察组,选取同期在我院进行健康体检的有正常健康生育史的男性志愿者52名为对照组。比较两组研究对象的精液参数(包括精液浓度、精子正常形态率、前向运动精子量)、精子顶体酶活性、精子DFI以及吸烟和生活作息情况;对影响精液不液化不育患者精子DFI的因素进行多因素logistic回归分析;对精液不液化不育患者精子DFI、精子顶体酶活性与其年龄、精液参数的关系进行Pearson相关分析;比较不同精子DFI精液不液化不育患者的体外受精结局。结果观察组患者的精液浓度、精子正常形态率、前向运动精子量、精子顶体酶活性显著小(少)于对照组,精子DFI、吸烟率、生活作息不规律发生率显著高于对照组,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,年龄、精液浓度、精子正常形态率、前向运动精子量、精子顶体酶活性、吸烟、生活作息不规律均是影响精液不液化不育患者精子DFI的独立因素。Pearson相关性分析结果显示,精液不液化不育患者的精子DFI与其年龄呈正相关,与其精液浓度、精子正常形态率、前向运动精子量、精子顶体酶活性呈负相关;精液不液化不育患者的精子顶体酶活性与其年龄呈负相关,与其精液浓度、精子正常形态率、前向运动精子量呈正相关。精子DFI<10%精液不液化不育患者体外受精的正常受精率显著高于精子DFI≥10%患者(P<0.05),但他们的胚胎卵裂率差异无统计学意义(P>0.05)。结论精液不液化不育患者的精子DNA碎片指数与其精子常规参数、精子顶体酶活性的关系密切,精子DNA碎片指数对患者的体外受精结局可能会产生重要的影响。
ObjectiveTo investigate the relationship between sperm DNA fragmentation index (DFI) and sperm conventional parameters, sperm acrosin activity in infertility patients with prolonged liquefaction and its effect on in vitro fertilization (IVF) outcome. MethodsA total of 106 infertility patients with prolonged liquefaction in our hospital from December 2018 to May 2020 were selected as the observation group. And 52 male volunteers with normal healthy fertility history who underwent physical examination in our hospital during the same period were selected as the control group. Semen parameters (including semen concentration, normal sperm morphological rate, progressive sperm quantity), sperm acrosin activity, sperm DFI, smoking and lifestyle condition were compared between the two groups. Multivariate logistic regression analysis was performed on the factors affecting the sperm DFI of infertility patients with prolonged liquefaction. Pearson correlation analysis was performed on the relationship between sperm DFI, sperm acrosin activity and age, semen parameters in prolonged liquefaction infertility patients. The IVF outcomes of prolonged liquefaction patients with different sperm DFI were compared. ResultsThe semen concentration, normal sperm morphology rate, progressive sperm quantity and sperm acrosin activity in the observation group were significantly lower than those in the control group, while sperm DFI, smoking rate and the incidence of irregular lifestyle were significantly higher than those in the control group, with statistically significant differences (P<0.05). Multivariate logistic regression analysis showed that age, semen concentration, normal morphology rate, progressive sperm quantity, sperm acrosin activity, smoking and irregular daily routine were all independent factors affecting sperm DFI of infertility patients with prolonged liquefaction. Pearson correlation analysis showed that the sperm DFI of infertility patients with prolonged liquefacient was positively correlated with age, and was negatively correlated with semen concentration, normal sperm morphology rate, progressive sperm quantity and sperm acrosin activity. Sperm acrosin activity in infertility patients with prolonged liquefaction was negatively correlated with age, and was positively correlated with the semen concentration, normal sperm morphology rate, progressive sperm quantity. The normal fertilization rate of IVF in prolonged liquefaction patients with sperm DFI<10% was significantly higher than that in patients with sperm DFI≥10% (P<0.05), but there was no statistically significant difference in their cleavage rate of embryos (P>0.05). ConclusionThe sperm DNA fragmentation index of infertility patients with prolonged liquefaction is closely related to their sperm conventional parameters and sperm acrosin activity, and the sperm DNA fragmentation index may have an important influence on the outcomes of in vitro fertilization.
