目的探讨共同决策模式干预在老年2型糖尿病(T2DM)患者饮食自我管理中的应用效果。方法选取2020年1月~10月收治的100例老年T2DM患者,将其随机分为对照组和观察组各50例。对照组患者给予常规饮食健康教育干预,观察组患者在对照组的基础上给予共同决策模式干预,两组干预时间均为6个月。比较两组患者干预前后饮食管理知识掌握程度和血糖代谢指标水平,以及干预后饮食行为依从性。结果干预6个月后,观察组患者的T2DM患者饮食管理知识量表得分和饮食行为依从性评分均高于对照组患者,外周血空腹血糖、餐后2小时血糖和糖化血红蛋白水平均低于对照组患者(均P<0.05),而治疗前上述指标(除饮食行为依从性测评量表评分外)两组间差异均无统计学意义(均P>0.05)。结论医护患共同决策模式干预可以显著地提高老年T2DM患者饮食管理知识掌握度和糖尿病饮食依从性,有助于改善患者的血糖控制效果。
ObjectiveTo explore the application effect of shared decision-making mode intervention on diet self-management in elderly patients with type 2 diabetes mellitus (T2DM). MethodsA total of 100 elderly patients with T2DM who were admitted to the hospital from January to October 2020 were selected, and they were randomly divided into a control group or an observation group, with 50 cases in each group. Patients in the control group were given a routine dietary health education intervention, based on which patients in the observation group were given an additional shared decision-making mode intervention, and both groups were intervened for 6 months. The mastery of diet management knowledge and blood glucose metabolism indexes levels before and after the intervention and dietary behavior compliance after the intervention were compared between the two groups. ResultsAfter 6 months of intervention, T2DM patients in the observation group scored higher on the diet management knowledge scale and dietary behavior compliance than those in the control group, while the peripheral blood fasting blood glucose, 2-hour postprandial blood glucose, and glycosylated hemoglobin levels of the observation group patients were lower than those of the control group patients (all P<0.05). There was no statistically significant difference in the above-mentioned indicators (except for the dietary behavior compliance assessment score) between the two groups before treatment (all P>0.05). ConclusionThe doctor-nurse-patient shared decision-making mode intervention can significantly improve the diet management knowledge mastery and the diabetic dietary behavior compliance in elderly T2DM patients, and it is helpful to improve patients′ blood glucose control effect.
