目的探讨幽门螺杆菌(HP)感染对原发性高血压(EH)患者血压、糖脂代谢和炎症反应的影响。方法选取2017年6月至2019年6月医院住院治疗的93例EH患者和89例行HP检测者作为研究对象,其中单纯EH患者49例(单纯EH组)、EH合并HP患者44例(EH合并HP组,高血压分级1级15例,2级16例,3级13例)、单纯HP患者43例(单纯HP组)、健康体检者46例(健康对照组)。比较治疗前上述四组间和单纯HP组与EH合并HP组组内治疗前后血压、空腹血糖(FBG)、血脂和C反应蛋白(CRP)水平,以及治疗前不同高血压分级EH合并HP患者的FBG、血脂和CRP水平。结果治疗前单纯EH组、EH合并HP组、单纯HP组患者的FBG、三酰甘油(TAG)、总胆固醇(TC)、CRP水平均高于健康对照组;EH合并HP组患者的收缩压、舒张压、FBG、TAG、TC、CRP水平高于单纯HP组,FBG、TAG、TC、CRP水平高于单纯EH组;在EH合并HP组患者中,FBG、TAG、TC、CRP水平均为高血压1级患者<2级患者<3级患者(均P<0.05)。HP感染根治治疗后,EH合并HP组患者的收缩压、舒张压、FBG、TAG、TC、CRP水平,以及单纯HP组患者的收缩压和舒张压均较治疗前低(均P<0.05)。结论HP感染对EH患者的血压、糖脂代谢和炎症反应有重要的影响,对EH合并HP患者进行HP根治治疗,对有效地控制血压、改善糖脂代谢、缓解炎症反应具有重要的临床意义。
ObjectiveTo investigate the effects of Helicobacter pylori (HP) infection on blood pressure, glucose and lipid metabolism, and inflammatory response in patients with essential hypertension (EH). MethodsA total of 93 hospitalized EH patients and 89 patients who went to the hospital for an HP test from June 2017 to June 2019 were selected as the research objects. Among them, 49 patients with pure EH (pure EH group), 44 patients with EH complicated with HP infection (EH complicated with HP group, 15 patients with stage 1 hypertension, 16 patients with stage 2 hypertension, and 13 patients with stage 3 hypertension), and 43 patients with pure HP infection (pure HP group), and 46 were healthy examinees (healthy control group). The blood pressure, fasting blood glucose (FBG), blood lipids, and C-reactive protein (CRP) levels were compared among the aforementioned 4 groups before the treatment and were compared in the pure HP group and the EH complicated with HP group before and after treatment; the FBG, CRP, and blood lipids levels were compared among EH complicated with HP infection of different blood pressure grades before the treatment.ResultsBefore the treatment, the FBG, triacylglycerol(TAG), total cholesterol (TC), and CRP levels were higher in the pure EH group, EH complicated with HP group, and pure HP group than in the healthy control group; patients in the EH complicated with HP group had higher systolic pressure, diastolic pressure, FBG, TAG, TC, and CRP levels than patients in the pure HP group, and higher FBG, TAG, TC, and CRP levels than the pure EH group; among patients in the EH complicated with HP group, the FBG, TAG, TC, and CRP levels increased in the sequence of patients with stage 1 hypertension, patients with stage 2 hypertension, and patients with stage 3 hypertension(all P<0.05). After the radical treatment for HP infection, the systolic pressure, diastolic pressure, FBG, TAG, TC, and CRP levels of patients in the EH complicated with HP group, as well as the systolic pressure and diastolic pressure of patients in the pure HP group were lower than those before the treatment(all P<0.05). ConclusionHP infection has important impacts on blood pressure, glucose and lipid metabolism, and inflammatory response in EH patients. HP radical treatment in EH patients complicated with HP has important clinical significance for effectively controlling blood pressure, improving glucose and lipid metabolism, and relieving inflammatory response.
