目的探讨华佗风痛宝片治疗骨关节炎大鼠软骨组织损伤的效果及其可能机制。方法选取6周龄SD大鼠随机分为假手术组、模型组、阳性组、高剂量组、低剂量组。假手术组大鼠,打开膝关节腔后直接缝合。其余组大鼠采用前交叉韧带切除术法制备膝骨关节炎大鼠模型。每组取10只存活的大鼠备用。阳性组大鼠给予壮骨关节丸治疗,高剂量组大鼠给予华佗风痛宝片18 g/kg治疗,低剂量组大鼠给予华佗风痛宝片4.5 g/kg治疗,模型组、假手术组大鼠给予蒸馏水灌胃。各组大鼠连续给药(治疗)8周。观察各组大鼠治疗后膝关节软骨组织形态,采用番红O-固绿染色于显微镜下观察各组大鼠膝关节软骨组织形态。比较各组大鼠治疗后血清肿瘤坏死因子(TNF)-α和基质金属蛋白酶(MMP)-13水平。比较各组大鼠治疗后膝关节软骨组织TNF-α、MMP-13、性别决定区Y框蛋白9(SOX9)和Ⅱ型胶原蛋白(CollagenⅡ)水平,以及连接蛋白43(Cx43)mRNA、蛋白相对表达水平。结果治疗8周后,与模型组比较,阳性组、高剂量组、低剂量组大鼠膝关节软骨组织磨损得到改善;模型组大鼠膝关节软骨基质染色不均,表层细胞缺失,有的成簇状,排列欠规则,染色稍浅淡,层次交错、紊乱,潮线不完整,钙化层增厚;高剂量组大鼠膝关节软骨组织细胞增殖明显,成簇状分布;低剂量组膝关节软骨组织细胞大小不等,有的成簇状,排列欠规则,层次稍紊乱但可辨认。治疗8周后,低剂量组大鼠血清 MMP-13水平低于模型组,高剂量组、低剂量组大鼠血清TNF-α水平均低于模型组;与模型组比较,高剂量组大鼠膝关节软骨组织SOX9 和CollagenⅡ表达升高, TNF-α、MMP-13表达减少,高剂量组、低剂量组和阳性组大鼠膝关节软骨组织Cx43 mRNA和蛋白相对表达水平均降低(均P<0.05)。结论华佗风痛宝片治疗骨关节炎大鼠效果较好,这可能与其能调节MMP-13、TNF-α、SOX9、CollagenⅡ和Cx43 水平有关。
ObjectiveTo investigate the effect and possible mechanism of Huatuo Fengtongbao Tablets on cartilage tissue injury in osteoarthritis rats. MethodsSix-week-old SD rats were randomly divided into sham-operated group, model group, positive group, high-dose group, and low-dose group. The rats in the sham-operated group were directly sutured after opening the knee joint cavities. The rats in the other groups were treated with anterior cruciate ligament resection to prepare rat models of knee osteoarthritis. 10 surviving rats were taken from each group for subsequent experiments. The rats in the positive group were treated with Zhuanggu joint pills. The rats in the high-dose group were treated with Huatuo Fengtongbao Tablets 18 g/kg, and the rats in the low-dose group were treated with Huatuo Fengtongbao Tablets 4.5 g/kg. The rats in the model group and the sham-operated group were given distilled water by gavage. Rats in each group were continuously administered (treated) for 8 weeks. The cartilage tissue morphology of knee joints of rats in each group was observed, and the knee joints cartilage tissue morphology of rats in each group was observed under a microscope with Safranin O-fast green staining. The serum levels of tumor necrosis factor (TNF)-α and matrix metalloproteinase (MMP)-13 were compared in each group after treatment. The levels of TNF-α, MMP-13, sex-determining region Y box 9 (SOX9) and type Ⅱ collagen (Collagen Ⅱ), as well as the relative expression levels of connexin 43 (Cx43) mRNA and protein in the cartilage tissue of knee joints of the rats in each group were compared after treatment. ResultsAfter 8 weeks of treatment, compared with the model group, the wear of the knee joints cartilage tissue of the rats in the positive group, the high-dose group and the low-dose group improved. In the model group, the cartilage matrix of the knee joints was stained unevenly, and the surface cells were missing, some of which were clustered and irregularly arranged, with slightly pale staining, interlaced and disordered layers, incomplete tidemark, and thickening of calcified cartilage zone. In the high-dose group, the cells proliferation of cartilage tissue in the knee joints of rats was obvious and distributed in clusters. In the low-dose group, the cells of cartilage tissue in the knee joints varied in size, and some of which were clustered, irregularly arranged and slightly disordered, but with recognizable layers. After 8 weeks of treatment, the serum MMP-13 level in the low-dose group was lower than that in the model group, and the serum TNF-α levels in the high-dose group and low-dose group were lower than that in the model group. Compared with the model group, the expressions of SOX9 and Collagen Ⅱ increased, and the expressions of TNF-α and MMP-13 decreased in the cartilage tissue of knee joints of the rats in the high-dose group, while the relative expression levels of Cx43 mRNA and protein in the cartilage tissue of knee joints of rats in high-dose group, low-dose group and positive group decreased (all P<0.05). ConclusionHuatuo Fengtongbao Tablets has a certain therapeutic effect on osteoarthritis rats, which may be related to its ability to regulate the levels of MMP-13, TNF-α, SOX9, Collagen Ⅱ and Cx43.
