目的探讨老年脑卒中患者急性期营养不良的影响因素及其风险预测模型的价值。方法选取192例老年急性期脑卒中患者,使用微型营养评估法评估患者的营养状态,并据此将患者分为营养不良组和非营养不良组。对老年脑卒中患者急性期营养不良的危险因素进行单因素分析和多因素logistic回归分析,构建老年脑卒中患者急性期营养不良风险预测模型并绘制列线图和受试者操作特征(ROC)曲线。结果192例老年急性期脑卒中患者中有59例(30.73%)出现营养不良。多因素logistic回归分析结果显示,年龄增加、低体质量指数(BMI)、焦虑、认知障碍、低血红蛋白水平、低白蛋白水平均是老年脑卒中患者急性期发生营养不良的独立危险因素(均P<0.05)。基于上述6项指标建立的老年脑卒中患者急性期营养不良风险预测模型,其Hosmer-Lemeshow检验结果显示,χ2=2.852,P=0.943;模型ROC曲线下面积为0.897,灵敏度为0.831,特异度为 0.820。结论老年脑卒中患者急性期营养不良的发生率较高,年龄增加、低BMI、焦虑、认知障碍、低血红蛋白水平、低白蛋白水平均是老年脑卒中患者急性期发生营养不良的独立危险因素;基于上述独立危险因素建立的老年脑卒中患者急性期营养不良的风险预测模型对老年脑卒中患者急性期发生营养不良有较好的预测能力。
ObjectiveTo explore the influencing factors of malnutrition in elderly acute stroke patients and the value of its risk prediction model. MethodsA total of 192 elderly acute stroke patients were selected to assess their nutritional status by Mini-Nutritional Assessment, according to which the patients were divided into a malnutrition group or a non-malnutrition group. Univariate analysis and multivariate logistic regression analysis were carried out to find the risk factors of malnutrition in elderly acute stroke patients, and a risk prediction model of malnutrition in elderly acute stroke patients was constructed, and the nomogram and receiver operating characteristic (ROC) curve were drawn. ResultsOf the 192 elderly acute stroke patients, 59 cases (30.73%) developed malnutrition. The results of multivariate logistic regression analysis showed that aging, low body mass index (BMI), anxiety, cognitive impairment, low hemoglobin level, and low albumin level were independent risk factors of malnutrition in elderly acute stroke patients (all P<0.05). A risk prediction model of malnutrition in elderly acute stroke patients was constructed based on the above 6 indicators, the Hosmer-Lemeshow test results showed that χ2=2.852, P=0.943; the area under the ROC curve of the model was 0.897, the sensitivity was 0.831, and the specificity was 0.820. ConclusionThe incidence of malnutrition in elderly acute stroke patients is quite high, and aging, low BMI, anxiety, cognitive impairment, low hemoglobin level, and low albumin level are independent risk factors of malnutrition in elderly acute stroke patients; the risk prediction model of malnutrition in elderly acute stroke patients constructed based on the above independent risk factors has good capability to predict malnutrition in elderly acute stroke patients.
